Q: I picked up a kit at an event, but don’t have a requisition form. How do I get one?

It’s very important that you send a sample back with a corresponding requisition form or the lab will not be able to process the sample. You can get this from your dashboard. If your patient is on any long-term drugs, just test as that is their new 'norm'. If it is a short course of, say, a couple of weeks, then wait two weeks and then test.
